Immunotherapy in ovarian cancer: ESGO publishes new first-line recommendations

Advanced epithelial ovarian cancer (stages III–IV) has a five-year survival rate below 30% with standard treatments. The emergence of PARP inhibitors and anti-PD-L1 antibodies has transformed the therapeutic landscape.

ESGO has published its 2025 recommendations for first-line treatment of epithelial ovarian carcinoma. All patients with high-grade serous carcinoma in stage III–IV should receive maintenance with a PARP inhibitor, regardless of BRCA mutational status, provided they have responded to first-line chemotherapy.

The role of immunotherapy

For patients with advanced or recurrent endometrial carcinoma with mismatch repair deficiency (dMMR), immune checkpoint inhibitors such as pembrolizumab have demonstrated significant survival benefit. ESGO recommends dMMR testing in all patients with advanced endometrial carcinoma before deciding systemic treatment.
