Spain has a caesarean section rate of 26%, above the 15% recommended by the WHO. SEGO published an updated clinical practice guideline in 2025 aiming to rationalise indications and reduce unnecessary caesarean sections.
Key updates
The new guideline establishes that elective caesarean at maternal request without medical indication should not be performed before 39 weeks and must be preceded by detailed information about risks and benefits. It also reinforces indications for VBAC in women with one previous caesarean and no specific contraindications.
In centres with continuous monitoring and on-call surgical teams, the VBAC success rate is 60–80%, with a uterine rupture risk below 1%.
